HOME-VISITING SCHEME

The Home Visiting Scheme offers services to older members of the community who for whatever reason have become, or are at risk of becoming isolated.

The Scheme aims to reduce social isolation, but also to promote awareness of local and borough wide information, support services and activities.

We can offer the following services

• Social visits at home on a regular basis including evenings and weekends
• Regular telephone contact
• Visits during a stay in hospital
• Practical support to ensure access to health and social care services.
• Help with paperwork or form filling etc.

Have you thought about becoming a volunteer?

We need volunteers with good communication skills, sensitivity and patience who can offer a small amount of time on a regular basis to visit an older person in their home. Could this be you? If so we’d welcome your call.
